Assists patients in the process of navigating post-acute care with an overall goal of creating a positive impact on patient outcomes and referral source satisfaction. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care. Represents the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.
As a LVN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.
The regional program director represents the region in activities involving professional contacts with physicians, hospitals, public health agencies, associations, executive level opportunities, and similar health groups and institutions, to apprise them of the availability of Medicare services.
The regional program director will collaborate with leadership to strategize for growth, and assist with implementation and execution of specialty programs and protocols that provide improved home health care services.
As Branch Director, you supervise the clinical operations of your team of field staff and report directly to the Agency Administrator. You will need to be organized, detail-oriented, and possess effective communication skills both orally and in writing. You will need to understand the issues related to the delivery of home health care services and be able to problem solve.
