In addition to performing visits and completing coordination of client care, the Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.

Our Physical Therapist Assistants work under the direction of our Physical Therapists and  treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. 

The Hospice Branch Director is a Registered Nurse who manages all aspects of terminal care and has expertise in pain and symptom management and previous experience in hospice nursing. Additionally, the Hospice Branch Director supervises the staff, has knowledge of and manages documentation requirements, oversees service coordination and case management. The Hospice Branch Director must have excellent communication skills and the ability to execute an effective, supportive sensitive approach in response to needs of the employees and the patients and their families.   The Hospice Branch Director supervises the day-to-day office and clinical operations as well as the planning, coordination, and delivery of the interdisciplinary team interventions. The Hospice Branch Director is responsible for the assignment of appropriate staff members to patients/families, and acting as a staff nurse when necessary. Also, the Hospice Branch Director participates in community awareness and education programs.

Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

Home Health with Less Travel!   Our Registered Nurses (RNs) supervise, evaluate, and provide nursing services to clients in various senior living communities, including assisted living and independent living facilities.   Our facility-based RNs enjoy: - providing care to our patients within the beautiful confines of the assisted living facilities where our patients reside - 1:1 patient care - the ability to follow each patient from start to discharge Enhabit RNs work collaboratively with facility wellness staff on a daily basis to ensure strong lines of communication and overall continuity of care for the residents and their loved ones. Enhabit Home Health & Hospice is looking for a Full-time weekend ]RN position. Enhabit Home Health & Hospice is searching for a Chaplain with hospice experience to join our team. The Chaplain is a member of the interdisciplinary team and reports to the Hospice Branch Director.  - Providing direct spiritual support and/or counsel to patients/families in keeping with patients/families’ beliefs. - Working with staff, clergy and community groups to enhance their sensitivity to the spiritual needs of patients/families and reporting on services as indicated. - Providing consultation and education for patients/families and interdisciplinary team members. - Providing bereavement follow-up services as defined by Hospice. - Maintaining proper records of visits to patients/families. - Performing occasional liturgical assignments such as the annual Service of Remembrance and monthly memorial service when indicated.

At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.   The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. At Enhabit Home Health & Hospice, an SLP may often work with patients who have experienced a stroke, and those with progressive diseases such as Parkinson’s disease and dementia.

Enhabit Hospice Nursing Aides assist with various personal care issues including changing bedding, changing clothing, bathing hospice patients and helping with hygienic routines such as brushing teeth, washing hair and keeping wound dressings clean and dry. Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs.
