As a LPN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Direct the planning and implementation of business development within the assigned division, including attainable forecasting. Identify and penetrate market opportunities, and provide strategic, in-field support for their team to achieve revenue and admission goals. Represent the company in activities involving professional contacts to apprise them of the availability of offered Medicare services. Monitor program execution and service provision. Manage the tenets of the program, develop professional relationships, and effectively support the company care teams.

Assists patients in the process of navigating post-acute care with an overall goal of creating a positive impact on patient outcomes and referral source satisfaction. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care. Represents the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.

Our Physical Therapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living.  We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program.  He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team.  These services are performed in accordance with the physician’s...

Enhabit Hospice Nursing Aides assist with various personal care issues including changing bedding, changing clothing, bathing hospice patients and helping with hygienic routines such as brushing teeth, washing hair and keeping wound dressings clean and dry. Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs.

As Branch Director, you supervise the clinical operations of your team of field staff and report directly to the Agency Administrator. You will need to be organized, detail-oriented, and possess effective communication skills both orally and in writing. You will need to understand the issues related to the delivery of home health care services and be able to problem solve. Previous home health and management experience will serve you well in this rewarding role, and an excellent knowledge of OASIS and Medicare guidelines is most helpful as you will be reviewing plans of care on a daily basis.

Our Speech Language Pathologists  supervise, evaluate and provide speech therapy services to clients in a home setting as prescribed by the attending physician.  The SLP treats patients with speech disorders, swallowing therapy, and hearing impairments that affect communication.  In addition they participate in helping patients that may be recovering from a stroke or other neurological disorders.

The Certified Occupational Therapy Assistant is a qualified professional who, under the supervision of the Director of Nurses and licensed Occupational Therapist, provides therapy services to clients in their homes under the Plan of Care established by the Occupational Therapist.
