The regional program director represents the region in activities involving professional contacts with physicians, hospitals, public health agencies, associations, executive level opportunities, and similar health groups and institutions, to apprise them of the availability of Medicare services.   The regional program director will collaborate with leadership to strategize for growth, and assist with implementation and execution of specialty programs and protocols that provide improved home health care services.   This position will be responsible for monitoring the execution of specialty programs and service provision through ongoing quality assurance visits with referral sources and working with their operational team members. The regional program director will be responsible for meeting and exceeding annual referral and admission goals as set by senior management, as well as assisting the regional sales team to do the same.

Our Physical Therapists (PTs) examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. Our Physical Therapists (PTs) use our Specialty Programs, including Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program to treat our patients.

Serve as the local chief executive agent. Lead the local provider(s) in a better way to care for the community in which it serves, and contribute to the overall success of the company. Ensure the execution of operations are completed daily. Manage operations, services, personnel performance, and office management ongoing.

In addition to performing visits and completing coordination of client care, the Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.

As Branch Director, you supervise the clinical operations of your team of field staff and report directly to the Agency Administrator. You will need to be organized, detail-oriented, and possess effective communication skills both orally and in writing. You will need to understand the issues related to the delivery of home health care services and be able to problem solve. Previous home health and management experience will serve you well in this rewarding role, and an excellent knowledge of OASIS and Medicare guidelines is most helpful as you will be reviewing plans of care on a daily basis.  

The community care regional program director (CCRPD) represents the region in activities involving professional contacts with senior living associations and living facilities, including assisted living facilities (ALF), independent living facilities (ILF), and personal care homes to apprise them of available Medicare services. Representation will also include rounding physicians and mid-level practitioners, private duty home care services, executive level opportunities, and similar health groups and institutions.   This position will collaborate with leadership to strategize for growth and assist with implementation and execution of community care programs that provide improved home health and hospice services.   The community care regional program director will monitor execution of the community care program and service provision through ongoing quality assurance visits with referral sources and working with their operational and clinical team members.   This position will be responsible to meet and exceed annual referral and admission goals as set by senior management.

Our Occupational Therapists help patients who have debilitating conditions improve the functions of performing everyday tasks in the home. Occupational therapists use treatments to develop the daily living skills of their patients, the basic motor functions of patients  as well as to compensate for any loss of function that may have occurred with the patient.

Our PRN Registered Nurse (RN) administers skilled care visits to clients requiring intermittent professional services and teaches the client, family, and other members of the health care team.  These services are performed in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the Branch Director.

As a LPN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.
