Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. At Enhabit Home Health & Hospice, an SLP may often work with patients who have experienced a stroke, and those with progressive diseases such as Parkinson’s disease and dementia.
Enhabit Home Health & Hospice is searching for a Hospice Volunteer Coordinator to join our team. The Volunteer Coordinator is responsible for all aspects of the hospice volunteer program and ensures the right volunteers are matched with the right families during the difficult transition.
- Recruit, select, train, and coordinate hospice volunteers.
- Design and deliver volunteer programs and services to meet patient, family and partner needs and enhance the customer experience
- Assign volunteers based on program needs and the volunteers’ interests and skills.
- Monitor and evaluate volunteers’ performance.
- Plan and conduct volunteer support meetings, volunteer appreciation gatherings, and volunteer retreats.
- Facilitate community awareness and support of Enhabit Hospice volunteer program.
- Assist Medical Social Work with community bereavement awareness as assigned.
As a Clinical Team Leader with Enhabit Home Health & Hospice, you will assist the Branch Director by ensuring the overall coordination of home health services to all clients is delivered according to acceptable standards of practice. The CTL is responsible for the electronic review and approval of all patient information submitted by the Licensed Professional (LP) during a Start of Care, Recertification, Resumption of Care, or Evaluation visit. The CTL assists with case conference, reviews and approves orders, in addition to providing oversight of patient care. The CTL is responsible for assisting the Branch Director with day-to-day office and staff management related to patient care and assists the Branch Director to provide ongoing education and training to all branch clinicians to ensure understanding of documentation requirements to meet regulatory standards. The CTL contributes to the overall company success of providing A Better Way ToCare by effectively facilitating the relationship between physicians, referral sources, patients, caregivers and employees. The CTL follows all procedures as outlined in Standard Operating Procedures (SOP).
The CTL role may require participation in an on-call rotation and making patient visits.
The licensed social worker administers social services to patients requiring intermittent professional services and teaches the patient, family, and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care.
Our Physical Therapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the Branch Director.
This part-time salaried position works 24 hours/week, over 4 days in the week, and is eligible for our 60% paid days off program.
As Branch Director, you supervise the clinical operations of your team of field staff and report directly to the Agency Administrator. You will need to be organized, detail-oriented, and possess effective communication skills both orally and in writing. You will need to understand the issues related to the delivery of home health care services and be able to problem solve. Previous home health and management experience will serve you well in this rewarding role, and an excellent knowledge of OASIS and Medicare guidelines is most helpful as you will be reviewing plans of care on a daily basis.

Enhabit Hospice Nursing Aides assist with various personal care issues including changing bedding, changing clothing, bathing hospice patients and helping with hygienic routines such as brushing teeth, washing hair and keeping wound dressings clean and dry. Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs.

This position assists with patient care review meetings, the review and approval of orders, and provides oversight of patient care.
