Overview

The Admission Coordinator is responsible for regional coordination of patient referrals. Act as a resource for referral sources, and work closely with branch staff to ensure that adequate coordination and transition occurs to hospice services. Occasional weekends and holidays required.

 

This is a fully remote position, however you must live in a state we do business in.

Responsibilities

  • Receive, document, and appropriately communicate referrals from physicians, facilities, and staff.
  • Gather and review required documentation for the hospice admission process.
  • Ensure orders for admission are clearly and appropriately documented in patient record.
  • Obtain and enter verbal orders from physicians and midlevel practitioners; ensure quality care coordination and timely entry into Homecare Homebase; if clinical.
  • Obtain agreement for admission from referred patients, and their care givers, who meet admission criteria.
  • Communicate status of referral clearly with referral sources, and any relevant branch staff.
  • Initiate contact with patient to verify awareness of referral, to introduce company, and to verify information; follow-up with the branch sales staff, as needed.
  • Manage and respond to calls from all branch clinical office staff regarding the care of patients.
  • Follow-up with patients who remain in the hospital or rehabilitation facility until current orders are received for admission; enter the readmission orders and follow-up with the appropriate branch.
  • Communicate with sales staff to coordinate referrals, specific requests, and dates of admission.
  • Complete all other duties as assigned.

Qualifications

  • Must have a high school diploma or equivalent.
  • Must have a current, unencumbered compact LPN/LVN license.
  • Must have one year demonstrated experience in hospice.
  • Demonstrated knowledge of Medicare guidelines governing hospice is required.
  • Must have basic demonstrated technology skills.
  • Experience must include typing and clerical activities.
  • Homecare Homebase experience is preferred.
  • Intake Coordinator/Admission experience in Hospice preferred.

Education and experience, preferred

  • A graduate of an approved school of clinical education, nursing or therapy, is highly preferred. If a graduate of a clinical school, must be licensed in the state in which they currently practice.
  • Three years' experience in hospice is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • If licensed, must be licensed in each state applicable to the required provision of services
